Valentine Hernandez is currently the Executive Director of QCCP Health Home and oversees all staff and operations of the Health Home. She is actively involved in the Coalition for New York State Health Homes and participated on the DSRIP steering committee for Mount Sinai PPS ,is a member of the Provider Advisory Council of Somos IPA, a voting member of the Executive Committee of New York Presbyterian Queens PPS and a Board Member of Venture House.
She has ten years of healthcare experience including: work in the field of HIV prevention and community health education, site administration of a Federally Qualified Health Center in Jamaica Queens and Health Home Leadership. Her first Healthcare related position was “grassroots” and community oriented and even included “hitting the streets” in a mobile unit to conduct HIV tests and conducting workshops hosted in prisons and shelters. As the Center Director for Community Healthcare Network’s center in Jamaica Queens she says the patients became like an extended family. Patients would scream ” Hi, Center Director” as she walked to the site each day. Her educational background includes graduating with honors completing a Bachelor of Arts in Women’s Studies from Fordham University and a Master of Arts from Lehman College in Community Health Education and Promotion with a specialization in program planning and evaluation. She holds Certificates in Executive Leadership for Healthcare professionals and Healthcare Leadership: Change Management from Cornell University Continuing Education Program.
Her passions include her religious community, humanitarian disaster relief volunteer work (such as Fordham Manor Reformed Church/ United4PR), HIIT and dance classes, creative writing and painting as art therapy.

Naresh Sallick is the Administrative Manager overseeing all financial aspects of Queens Coordinated Care Partners. During his tenure (in collaboration with the governing Finance Committee) he developed the financial infrastructure of QCCP Health Home. He is responsible for maintaining financial viability of the Health Home entity as well as, assisting contracted Case Management Agencies address issues that may affect their internal viability. In his role, he oversaw the implementation of the Direct Billing transition in 2017 and has implemented an effective process resulting in a 1 to 3 day turnaround for disbursing payments. Prior to joining QCCP, Naresh spent the majority of his career as a financial analyst with Mount Sinai Health Partners. He earned his degree in Business and Accounting at Queens College. He has obtained a certificate in Continuing Educations from Baruch College (Communicating in Finance and Accounting) and is currently working towards certifications in (Financial Management, Hospitality Management, Master Certificate in Revenue Management) from Cornell University Continuing Education Program. His passion is entrepreneurship.

Oscar A. Laluyan is the Program Assistant for QCCP and oversees transfers, referrals, ED alerts, manages schedules of the team, and other daily administrative duties. He has worked in Health Homes since 2013 and began as a data entry clerk. He actively collaborates with many of the care management agencies for them to become pro-active problem solvers. His past professional experiences include, a medical biller for a physical therapy company, a corporate communications specialist for a financial rating agency, and a program coordinator for a New York museum. Among many of Oscar’s accomplishments at QCCP include implementing the ED Alerts trackers and helping realize the first of many annual QI Symposiums to come! Oscar’s educational background includes studying Fine Arts in college. The variety and breadth of his work experience has proven to be a dynamic advantage. His interests include fine art, writing, books, and he is an avid trivia buff.
